Frontal Lobe — Cerebral Cortex

The frontal lobe is the largest lobe of the brain and is responsible for some of our most distinctly human capabilities. It governs complex thought, decision-making, personality expression, and voluntary movement. The prefrontal cortex - its foremost region - plays a central role in executive function and social behaviour.

Anatomy & Location

The frontal lobe sits directly behind the forehead and is the largest of the brain's four paired lobes, accounting for roughly a third of the cerebral cortex's surface area. It's bounded at the back by the central sulcus, a deep groove that separates it from the parietal lobe, and at the bottom by the lateral (Sylvian) fissure, which separates it from the temporal lobe. Its rearmost strip, the primary motor cortex, sits right along the central sulcus, while everything in front of that - collectively called the prefrontal cortex - handles the lobe's higher-order work.

Function in Depth

The frontal lobe is often described as the brain's "command centre" because it coordinates so many processes that feel like the essence of being a person: planning, impulse control, working memory, judgment, and voluntary movement. The primary motor cortex sends direct signals down the spinal cord to initiate deliberate muscle movement, organised somatotopically along a strip often illustrated as the "motor homunculus" - a distorted body map where the hands and face take up disproportionately large territory because of the fine control they require.

In front of the motor strip, the prefrontal cortex handles executive function: weighing consequences, delaying gratification, switching between tasks, and regulating emotional responses generated elsewhere in the brain (particularly by the amygdala). It's also heavily involved in personality and social behaviour - our sense of appropriateness, empathy, and self-control are largely mediated here. Broca's area, typically in the left frontal lobe, is responsible for producing fluent, grammatically organised speech.

Notable Research & History

No case has shaped our understanding of the frontal lobe more than that of Phineas Gage, a railroad foreman who in 1848 survived an iron rod being driven through his skull in a construction accident, destroying much of his left frontal lobe. Gage lived another twelve years, but by contemporary accounts his personality changed markedly - he reportedly became more impulsive and socially unrestrained. While modern historians debate how dramatic and permanent the change really was, the case remains the foundational example linking the frontal lobe to personality and self-regulation, and it helped launch the field of behavioural neurology.

In the mid-20th century, the frontal lobe was also the target of prefrontal lobotomy, a now-discredited psychosurgical procedure once used to treat severe mental illness. Its serious, often devastating side effects on personality and cognition - learned partly from cases like Gage's - contributed to its abandonment once effective psychiatric medications became available.

When Something Goes Wrong

Frontal lobe dysfunction can arise from traumatic brain injury, stroke, tumours, or neurodegenerative disease, and its effects vary enormously depending on which part is affected. Damage to the prefrontal cortex can impair judgment, planning, and impulse control without necessarily affecting memory or language. Damage further back, near the motor strip, can cause weakness or paralysis on the opposite side of the body. The frontal lobe is also implicated in conditions such as ADHD, where executive function circuits develop or operate differently, traumatic brain injury, depression, and certain forms of stroke-related language impairment when Broca's area is involved.
