Schizophrenia is a complex psychiatric condition, not the "split personality" it's sometimes popularly (and incorrectly) confused with - it involves disruptions to thinking, perception, and motivation rather than multiple personalities. Symptoms are often grouped into three categories: positive symptoms (hallucinations and delusions - experiences added on top of typical functioning), negative symptoms (reduced motivation, emotional expression, or social engagement), and cognitive symptoms (difficulty with attention, working memory, and organising thought). Symptoms most often emerge in late adolescence or early adulthood.

Affected Brain Regions

The frontal lobe shows altered activity related to the negative and cognitive symptoms of schizophrenia, consistent with its role in executive function and motivation. The temporal lobe, including its auditory processing areas, has been linked to auditory hallucinations, and memory-related temporal structures are implicated in some of the cognitive symptoms. The thalamus's role as the brain's sensory relay and filter has led to a long-standing theory that disrupted thalamic "gating" - a failure to filter out irrelevant sensory and internal information - may contribute to symptoms like hallucinations and disorganised thinking.

Current Research Directions

Antipsychotic medications, most of which work by modulating dopamine signalling, remain the primary treatment, built on a decades-old "dopamine hypothesis" that has since been substantially refined. More recent research has increasingly focused on the glutamate neurotransmitter system as another key piece of the puzzle, and on early-intervention programs aimed at identifying and treating first episodes as early as possible, since earlier treatment is associated with better long-term outcomes. Large-scale neuroimaging studies increasingly frame schizophrenia as a disorder of brain network connectivity rather than damage to any single region.
