Overview

Anxiety disorders are an umbrella category - including generalised anxiety disorder, panic disorder, social anxiety disorder, specific phobias, and post-traumatic stress disorder - united by fear or worry that is disproportionate to the actual situation, persistent, and significant enough to interfere with daily functioning. Anxiety itself is a normal, adaptive response; what distinguishes an anxiety disorder is its intensity, persistence, and impact, not the presence of anxiety itself.

Affected Brain Regions

The amygdala, the brain's threat-detection hub, characteristically shows heightened reactivity to potential threat cues in anxiety disorders, contributing to an exaggerated alarm response. The frontal lobe's prefrontal cortex normally helps regulate and dampen this response, and reduced regulatory activity there is thought to weaken that natural braking mechanism, allowing anxious reactions to persist or escalate. The hypothalamus, at the top of the body's stress-response (HPA) axis, is often chronically activated in anxiety disorders, contributing to the physical symptoms - rapid heartbeat, muscle tension, fatigue - that frequently accompany psychological anxiety.

Current Research Directions

Cognitive behavioural therapy and exposure-based approaches, which help retrain the brain's threat-response patterns through structured practice, remain the most evidence-backed non-medication treatments, and neuroimaging studies have helped clarify how they alter amygdala-prefrontal circuit activity over the course of treatment. Medications affecting serotonin and norepinephrine signalling are commonly used alongside therapy. Research continues into more personalised treatment matching, given how differently anxiety disorders present and respond to treatment across individuals.
