A chronic autoimmune condition in which the immune system attacks myelin, the insulating sheath around nerve fibres in the brain and spinal cord, disrupting communication between the brain and the rest of the body.

Overview

Multiple sclerosis (MS) is a chronic autoimmune condition in which the immune system mistakenly attacks myelin, the fatty insulating sheath that wraps around nerve fibres in the brain and spinal cord and allows electrical signals to travel quickly and efficiently. The resulting damage - and the scar tissue ("sclerosis") it leaves behind - can occur almost anywhere in the central nervous system, which is why MS symptoms vary so widely between individuals. Most people are diagnosed with a relapsing-remitting course, with distinct symptom flare-ups followed by partial or full recovery, though the disease can become more steadily progressive over time.

Affected Brain Regions

Because MS attacks myelin wherever it occurs, the corpus callosum - the brain's densest bundle of myelinated fibres - is one of the most common and characteristic sites of MS lesions on brain imaging. Lesions in the brain stem can affect eye movement, facial sensation, and swallowing, given the concentration of cranial nerve pathways passing through it. The cerebellum and its connections are also frequently affected, contributing to the balance and coordination problems common in MS, which can resemble cerebellar ataxia from other causes.

Current Research Directions

Treatment has advanced substantially with a growing range of disease-modifying therapies that reduce immune-driven attacks on myelin and lower relapse frequency, though no treatment yet reverses existing damage. A major focus of current research is remyelination - finding ways to encourage the nervous system's own limited capacity to repair damaged myelin - alongside better biomarkers for earlier diagnosis and for predicting which patients are likely to have a more aggressive disease course.
